Owner & Licensed Physical Therapist
When I took over Golden Gate Physical Therapy in 2004, I envisioned a clinic with a sense of ohana, which means family in Hawaiian. I believe every patient should be treated like they are a part of our family, and this is the welcoming spirit that guides our patient care every day. Your health and well-being are important to us and we want to give you the careful, personalized attention you deserve.
We are proud to be the exception among San Francisco physical therapy clinics, devoting 100% professional attention to every patient at every visit with licensed physical therapists.
If you’d like to know more about how we might approach treatment of your specific physical issue or injury, we invite you to schedule a free phone consultation with one of our licensed physical therapy experts.
We make completing your initial paperwork simple and flexible. Please choose the option that is most convenient for you:
Please arrive 10–15 minutes prior to your appointment time to allow ample time to complete the intake forms in our comfortable reception area.
Download and print the forms at home, complete them at your own pace, and bring them with you to your first visit.
Fill out our fillable PDF forms electronically and email them directly to our front desk team prior to your appointment.
Access fillable new patient medical history questionnaires and intake documents.
Read our HIPAA privacy policy for detailed information on how we protect your health information.
We know insurance can feel complicated. We pride ourselves on clear communication and transparent claim filing so you can focus 100% on your recovery.
Although it may take 30–60 days to receive a bill for your deductible from your insurance processor, all co-pays are expected at the time service or treatment is provided.
As a courtesy to our patients, Golden Gate Physical Therapy will file claims with patients’ PRIMARY insurance company for services rendered and help patients get direct payments from the insurance company.
We do not bill secondary or tertiary insurance directly, but we will provide all necessary billing codes, clinical notes, and documentation for patients to submit. Ultimately, patients are responsible for their charges.
We accept select PPOs. Please contact our office for more info and to verify your specific coverage and benefits.
We are participating Medicare providers and accept Medicare Part B for physical therapy rehabilitation.
The first step in this process is making your initial evaluation appointment, which will last 45 minutes with one of our physical therapists. Once scheduled and your insurance information is provided, we verify benefits and follow this transparent process:
We send your visit charge per session (determined by your licensed physical therapist based on clinical treatment provided) directly to your insurance company.
We receive an Explanation of Benefits (EOB) within 30–45 days from your insurance company outlining allowable amounts, covered portions, and deductibles.
After claims are fully processed and paid by your insurance company, you will receive a transparent statement from GGPT for any remaining balance due.
We accept select PPOs. Please contact our office for more info and to ensure that you understand your coverage, visit limits, and claim procedures.
